The areola and nipple

Reconstruction of the nipple and areola (Nipple- Areolar Complex, NAC) is the last stage of a breast reconstruction.

The position of the areola and nipple cannot be done with precision unless the two breasts are sufficiently symmetrical; because once it is done the areola is difficult to move.

While waiting for this reconstruction, adhesive areolas offer a good choice of shapes and colours, with a very natural effect. These areolas can in particular be used under a swimsuit.

The areola

There are two techniques to reconstruct an areola.

Tattooing

  • Local anaesthesia.
  • For pale areolas, it is often preferable to do a bilateral tattooing in order to have the same colour on both sides.
  • Retouches are sometimes necessary to obtain a better result or maintain a result over time.
  • This technique offers good results, but some women say that it gives a rather 'smooth' appearance.

Skin graft

  • Local or general anaesthesia.
  • The skin sample is taken from the groin area.
  • The results are good, but it leaves a scar in the groin (often inconspicuous).

The nipple

Usually reconstructed at the same time as the areola. There are three possible techniques.

  • Part of the nipple on the other (contralateral) breast is grafted, provided that it is not too small to do so. The graft does not leave any visible scar, nor affect sensitivity.
  • Graft of part of the labia minora.
  • A small local flap: a skin graft is then carried out to reconstruct the areola.

It is always possible to carry out a breast reconstruction, even years after removal.
In the absence of radiotherapy, reconstruction can take place 6 months after the mastectomy (after tissues are well healed).

In the case of radiotherapy, it is preferable to wait for one year. In the meantime, it is recommended to massage the chest area of the breast to soften the tissues.
